Silver Linings for Seniors can provide:

• An initial meeting with client and family to determine appropriate customized solutions and ongoing care management to monitor client and update support services as needed.
• A one-time consultation which includes a needs assessment with recommendations.

• Liaison between client, family and all healthcare and service providers
• Assist with financial and healthcare paperwork and applications
• Suggest and coordinate home modifications to improve safety
• Help to avoid unnecessary hospitalizations or inappropriate long-term placement
• Advocate for client during hospital stays, discharge planning and rehabilitation
• Explore housing options, including assisted living and long-term care, and coordinate re-location.
• Supplement existing services whether your loved one is at home or in a facility setting.
• Support and guide client and family when making end-of-life decisions.
